To be eligible for Adult Day Care Services, the individual must be:
- Elderly (unable to stay at home alone at risk of losing balance, falling down or cannot independently care for himself or herself)
- Youth with Special Needs
- Adults with Special Needs
- Developmentally Disabled
You do not need a recommendation from a physician to be eligible for adult day care service but referrals are more than welcome. If you wish to refer your friend, your family member or one of your patients to our services, please use our online form.
